How to Schedule a Holistic Referral Case

• Please note that we will ask you to have your veterinarian fax a copy of the relevant medical records, before we will confirm an exact appointment time.
• Recently, many referral clients have initially faxed us computerized invoices of services rendered. These invoices do not contain the prior diagnosis (sometimes many), treatment plans, or laboratory results/interpretations. These are not the doctor's medical records. It will save the client and Dr. Johnson's technician considerable time, if the doctor's medical records are sent the first time around. 
• If you have seen several veterinarians with your pet, we sometimes need the records from each one. The last two years of records will usually suffice.
